Lauren Falcone, MSN, AGPCNP-BC

Lauren Falcone, MSN, AGPCNP-BC, has been a dedicated provider at Elevate Health and Wellness since 2021, offering thoughtful and personalized medication management for adolescents and adults. She takes a collaborative, patient-centered approach that blends evidence-based care with a holistic perspective, ensuring each treatment plan is tailored to the individual’s unique needs, goals, and lifestyle.

Lauren is especially passionate about working with women navigating late ADHD diagnoses, as well as supporting patients through the perinatal and postpartum periods. She treats a wide range of conditions including depression, anxiety, ADHD, and mood disorders, providing care both in-person and via telehealth.

She earned her Bachelor’s degree in Psychology in 2007 and her BSN in Nursing in 2012. Prior to becoming a nurse practitioner, Lauren spent seven years working in a high-volume emergency department in Brooklyn, NY, where she developed strong clinical judgment, resilience, and a deep appreciation for compassionate care. She completed her Master of Science in Nursing in 2019 and transitioned into psychiatric practice soon after.

Known for her warm demeanor and approachable sense of humor, Lauren strives to make patients feel comfortable, empowered, and actively involved in their treatment decisions, fostering a therapeutic environment built on trust, transparency, and partnership.
